Please note that the Practice does not accept Medicaid, and patients requiring a referral will need to call the office or submit an online appointment request here to have one of our staff members call you to schedule an appointment.

All co-pays will be collected at check-in. A $200 deposit will be collected at check-in for all self-pay patients. Fees for cosmetic services are due in full at the time of service.
